Thank you for your interest in participating in the Adopt-a-Student program offered by the Michigan Pharmacy Foundation. This program is designed to support connections between student pharmacists and practicing pharmacists during the Annual Convention & Exposition (ACE).

Please complete this brief questionnaire to share some general information about yourself. Your responses will help the Foundation facilitate mentor engagement as part of the program.

The form should take approximately 5 minutes to complete.
