Parent Questionnaire

Welcome to the Adopt-a-Student 2026 Mentor Program!

Thank you for your interest in supporting the next generation of Michigan pharmacists. This program pairs pharmacists (“Adoptive Parents”) with student pharmacists attending the Annual Convention & Exposition (ACE). 

As a mentor, you’ll have the opportunity to share your experience, offer encouragement, and connect with students in a meaningful way throughout the convention.

Please complete this brief questionnaire to help us introduce you to students.  Your responses will be shared in a catalog that students can browse when selecting their mentor, so feel free to highlight your background, interests, and any areas where you’d enjoy offering guidance.

The form should take about 5 minutes to complete.
